Acceptance of these terms
These Terms & Conditions govern your use of the website at healinghorizons.us, operated by Healing Horizons Psychiatry (“we,” “us,” or “our”), a psychiatric practice with clinics in Glendale and Phoenix, Arizona.
By browsing this site, submitting a form, or using our online payment portal, you agree to these terms. If you do not agree with them, please do not use the site — you are always welcome to contact the practice by phone instead.
This website is not medical advice
The content on this site — including descriptions of conditions, treatments, services, and any educational material — is provided for general information only. It is not a substitute for a professional evaluation, diagnosis, or treatment by a qualified clinician who knows your individual circumstances.
Never disregard professional medical advice, or delay seeking it, because of something you read on this website. Reading this site does not create a provider-patient relationship. That relationship begins only once you are formally accepted as a patient and have attended an appointment with one of our clinicians.
Appointment requests and communication
Submitting the appointment request form is a request only — it is not a confirmed booking. A member of our care team will contact you, usually within one business day, to confirm availability, verify your insurance, and schedule you. An appointment exists only once we have confirmed it with you directly.
Please provide accurate contact details so we can reach you. Email and web forms are not secure channels; do not use them to share detailed clinical information, and expect us to move sensitive conversations to a phone call or your appointment.
Appointments, cancellations, and no-shows
We reserve a dedicated block of clinician time for every appointment. So that we can offer that time to another patient if you cannot attend:
- Please give at least 24 hours' notice to cancel or reschedule
- Cancellations with less than 24 hours' notice, and missed appointments, may incur a fee that insurance does not cover
- Repeated missed appointments may affect our ability to continue offering you scheduled care
- Arriving significantly late may mean your appointment has to be shortened or rescheduled
We understand that emergencies happen. If something unavoidable comes up, call us at 480-393-3515 and we will do our best to work with you — fees are applied at our discretion, not automatically.
Fees, billing, and online payments
You are responsible for all charges for services provided to you, including copayments, coinsurance, deductibles, and any balance your insurance plan does not cover. We verify your benefits before your first visit and will tell you what to expect, but a benefits verification is an estimate — not a guarantee of coverage or payment by your insurer.
Online payments are processed through YourHealthBill (powered by Raxia), a third-party payment provider, on their own secure systems. Their terms and privacy policy apply to that transaction. We do not receive or store your full card number or bank account details. You are responsible for making sure the payment details you enter are accurate.
Balances not paid within a reasonable period may be referred to a collections agency. We would much rather talk to you first — if you are struggling to pay, call our billing team at 480-393-3515 to discuss a payment plan or self-pay options.
Acceptable use of this website
When using this website, you agree not to:
- Submit false, misleading, or another person's information without their authority
- Attempt to gain unauthorised access to any part of the site, our systems, or another person's data
- Interfere with the operation of the site, including by introducing malicious code or automated scraping
- Use the site for any unlawful purpose or in violation of these terms
- Copy, republish, or commercially reuse our content without our written permission
We may suspend or restrict access to the site for anyone who breaches these terms.

All content on this website — text, graphics, logos, photographs, layout, and design — is owned by Healing Horizons Psychiatry or used under licence, and is protected by copyright and trademark law. This site links to services we do not control, including our payment portal and our social media pages. We provide these links for your convenience. We are not responsible for the content, availability, security, or practices of any third-party site, and a link does not imply our endorsement of everything found there. Your use of those services is governed by their own terms.
Website availability and accuracy
We work to keep this site accurate and available, but we provide it “as is” without warranties of any kind. Service descriptions, accepted insurance plans, clinician availability, hours, and fees can change without notice. We may modify, suspend, or withdraw any part of the site at any time. Always confirm current details with our office before relying on them.
Limitation of liability
To the fullest extent permitted by Arizona law, Healing Horizons Psychiatry and its staff are not liable for any indirect, incidental, or consequential damages arising from your use of, or inability to use, this website — including reliance on its general information. Nothing in these terms limits our responsibility for the professional care we provide to our patients, or any liability that cannot lawfully be excluded.
Governing law
These terms are governed by the laws of the State of Arizona, without regard to its conflict of law rules. Any dispute relating to this website or these terms will be brought in the state or federal courts located in Maricopa County, Arizona.
Changes to these terms
We may update these Terms & Conditions from time to time. The current version will always be posted on this page with its “last updated” date. Continuing to use the site after a change takes effect means you accept the revised terms.
