Healing Horizons Psychiatry (“we,” “us,” or “our”) is a psychiatric practice with clinics in Glendale and Phoenix, Arizona. We understand that the information you share with us — about your mental health, your history, and your life — is deeply personal. Protecting it is a core part of the care we provide.
This Privacy Policy explains what information we collect through our website at healinghorizons.us, how we use it, and the choices you have. It also summarises how we handle protected health information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA).
Information we collect
Information you give us directly. When you submit our appointment request or contact form, we collect the details you enter:
- Your first and last name
- Your email address and, if you provide it, your phone number
- Your reason for visit and preferred contact method
- Anything you choose to write in the optional message field
Information collected automatically. Like most websites, our hosting provider records basic technical data such as your IP address, browser type, device type, the pages you visit, and the time of your visit. This is used for security and to understand which pages are useful.
Information from your care. Once you become a patient, we maintain a clinical record including your medical and psychiatric history, diagnoses, treatment plans, medications, session notes, and insurance and billing details. This information is not collected through this website.
How we use your information
We use the information described above only to:
- Respond to your appointment request, question, or billing enquiry
- Schedule, confirm, and remind you about appointments
- Verify your insurance benefits and obtain any prior authorisations needed
- Provide, coordinate, and document your psychiatric care
- Bill your insurance plan and collect payment for services
- Meet our legal, regulatory, and professional obligations
- Keep our website secure and improve how it works
We do not sell, rent, or trade your personal or health information. We do not use your health information for advertising, and we do not share it with advertisers or data brokers.
Your privacy rights
Under HIPAA and Arizona law, you have the right to:
- Inspect and obtain a copy of your medical and billing records
- Request a correction to information you believe is inaccurate or incomplete
- Request that we communicate with you in a specific way or at a specific location
- Request restrictions on how we use or disclose your information

How we protect your information
We maintain administrative, physical, and technical safeguards designed to protect your information. These include encrypted connections (TLS/SSL) across our website and patient portal, access controls that limit staff access to what each role requires, staff training on privacy obligations, and contractual protections with every vendor that touches your data.
Online payments are processed by YourHealthBill (powered by Raxia) on their own PCI DSS compliant systems. We never see or store your full card number or bank account details.
No method of transmission or storage is completely secure. While we work hard to protect your information, we cannot guarantee absolute security — which is why we ask you not to send sensitive clinical details by email or web form.
How long we keep information
We retain medical records for the period required by Arizona law and professional standards — generally at least six years for adult patients, and for minor patients until at least three years after they reach the age of majority. Website enquiries that do not become patient records are kept only as long as needed to respond and to maintain a record of the request. When information is no longer needed, it is securely destroyed.
Children's privacy
We provide care to children and adolescents, and we treat their information with the same protections described here plus the additional safeguards Arizona law affords to minors. Our website is not directed at children, and we do not knowingly collect information online from a child without a parent or guardian. If a parent or guardian believes a child has submitted information through this site, please contact us and we will remove it.
Links to other websites
Our site links to third-party services, including our payment portal at yourhealthbill.com and our social media pages. Once you follow such a link you are subject to that organisation's own privacy policy, not this one. We encourage you to review the policy of any site you visit.
Changes to this policy
We may update this Privacy Policy to reflect changes in our practices or in the law. The revised version will be posted on this page with a new “last updated” date. Material changes affecting how we handle your protected health information will also be reflected in our Notice of Privacy Practices, available at our clinics.
